You have much to learn before we can even begin on people paying for the labor and material investments of medical personnel.

The other thing that causes our healthcare to get screwed up is poor people use the ER as a place to deal with minor shit. I used to know a PA in Baltimore who told me that certain demographics would take their children to the ER for colds, or to check them for bedbugs, or they would go in for a sore throat. This rages the staff at hospitals, who are using scarce expensive resources on shit that is not life threatening, and it won't change the outcome much. Think about it: If you have a cold, what fucking difference does it make if you go to the hospital. The doctor there will tell you take nyquil or whatever, sleep, and maybe eat some Vitamin C. Its not like you won't be sick anymore. Yet countless people do this shit every day in his city. Or they will come in saying they have pain, and then write on the form they are alleargic to tylenol, aspirin, and all the other non narcotic painkillers. Other folks used the ambulance as a taxi service, and their illness would magically disappear at the hospital and they'd get out there as it was a lot closer to their destination. Many of these folks who use the hospital for these non legitimate purposes also do not pay the bill, which means that the paying people's costs are increased to make up the difference.
Using the hospital for these purposes is kind of like giving a person who is hungry, a pretzel or hot dog from a football stadium. Its not going to be that great, but it will be expensive, and other forms of food are better for you and cheaper. The system is fucked, but there are a great deal of poor people who abuse the shit out of our nation's hospitals and take advantage of the staff, and that is a problem that very few leftist politicians have addressed.

The government pays the bill. But the government does not tell doctors what to do. The doctor decides what to do based on their medical training. They are held accountable for their clinical decisions by their conscience, their patients and their professional association.
