You ignoring the orderly high trust society needed to grow this civilization, and that it doesn't exist in places like say, Sudan. It isn't the case that a small number of people do the lifting, and everyone else plays no role.

The entire world is being dragged along by the West and to a lesser extent the north Asian nations (China, South Korea, Japan). That's it, dude.

It's not just technology and medicine either. It's food. The rest of the world never developed the adaptations that allowed them to get out of the Malthusian trap. The only reason they have the populations they do is because we keep feeding them with our surplus yields.
