Only one arm of the TRIAD is available, the bombers, the Americans can't risk a trajectory launch towards Russia and China, and even with the bombers, realistically, the only option is counterforce, with B61, and in fact, B61 is the only weapon in the arsenal for the bombers, other than AGM-86, and since AGM-86 simply adds range to the warhead, and North Korea is well within the radius of action for B61, there would be no reason to waste AGM-86's on it, save those for the Russians.

Depends upon the yield, altitude of detonation, and geographic features, I'd imagine. Over relatively flat terrain, assuming something like they just detonated, it's pretty fucking far. The Congressional report on the subject estimated only a handful of bombs would be needed to send the entire continent back into the 18th century.

Well, they popped Starfish Prime over Johnston Island, and it blew the grid in Hawaii, so that's about a thousand miles, so three of them would probably cover the CONUS.
