I would hesitate to use the word system to describe the Faultocracy, but for the sake or argument, I can let it stand. "System" almost implies intelligent design, or at least a coherent system that is a result of millions of years of evolutionary processes that weed out stupidity.

We are talking about a "system" that builds a suspension bridge without the suspension cables attached, fucked even that up, and then decided to tighten tension cables in the span while human beings were underneath, and it all collapsed.

We can blame lack of regulations all day, but at the end of that day, this was monumental stupidity fueled by democratic party corruption.

Just need to hold everyone with responsibility in the project to account for what they did and did not do............. civilian contractors and government agents alike. But that's not what's going to happen. The government will rush to prosecute the civilians and protect the government from any responsibility.
