First strike is irrelevant unless you can take out 100% of the nukes. Whatever we have left would be glassing half the globe in response.

I don't think it will even be clear who struck America when it comes
in order for MAD to work, you have to know who hit you from where
it's not going to be clear, just like 9-11
it's going to come out of the blue from an unexpected vector, covert and asymmetrical
if someone pops a multimegaton nuclear torpedo up the Potomac
there is no track as to where that came from, nor who launched the attack'
furthermore, the people in command would all be dead, or out of contact

this is not that war anymore, the attack will be covert, asymmetrical and by proxy
you won't have any targets to retaliate against, because you won't know who hit you from where
maybe it is a Pakistani nuke, but it could be terrorists delivering it by sea container
