Great scene. Basically Robson got kicked out of the Aryan Brotherhood because he got a gum transplant from a black man. As a result, he was by himself and the Italians and Muslims tried to kill him. So he sought out Wolfgang who gave him protection in exchange for sexual slavery. The spoon was inserted into his anus to get him used to the humiliations he would endure from then on. Really powerful stuff.
