The best way to do it, in my humble opinion, is using a few of those supercavetating torpedoes the Russians have. Those torpedoes actually fly pretty deep beneath the surface, at speeds faster than we can easily intercept. A few of those with nuclear warheads would do the job pretty easily.
I am unsure about their range. Range would have to be really far because we sweep pretty far around a carrier group and will hunt down subs within that range.

But again, a submarine is going to get a lot of attention, the US Navy knows those NK Romeo SSK's by their acoustic signatures, getting close with a submarine is actually trickier than getting close with a commercial vessel in a chokepoint; just get lost in the crowd, and then come at them undercover as a "civilian", just like Iraqistan, except at sea.

The SSN's actually have trouble with commercial vessels in a chokepoint, it's an acoustic nightmare for them in a maritime traffic jam, this is how and why American SSN's have actually collided with commercial fishing vessels on several occasions, the SSN is made to hunt the big stuff, warships and submarines, but they ain't set up to hunt fishermen, fishermen are actually the bane of their existence, hitting a fisherman is actually the thing they worry about the most, mostly because it's a career ender, with possible jail time, but they won't be able to spot one with a nuke, just by sonar and periscope.
You keep the nuke slung in a moonpool, you're only going to trail it for the final approach, they have fishing boats now where you fish through a moonpool in the bottom of the boat, but they wouldn't be able to "see" it either way, that's the whole point, hide in plain sight is the easiest way to approach the US military, easier than using a submarine, which they are on high alert for.
They could even flag it as a "Russian" trawler, then the Americans would be suspicious, but wouldn't dare sink it on approach, because they'd think it was a Russian intelligence gathering vessel, but then when it pops, maybe the Russians get blamed, it certainly would confuse the matter, but there's no reason to make it easy to identify, they don't have to fly the DPRK flag.
Doesn't even have to be overt "retaliation", they could just sow complete chaos and send the US & Co into a frenzy, without actually leaving a calling card that said "If found, return to Kim Jong Un, North Korea", you would know that it might have been them, but a clear mandate to retaliate, they may not even provide, they, along with the Russians and Chinese, would of course blame the USA, for "one of its own" nukes going off, in a "catastrophic disaster".

The Russians are the perfect cover for the NK's to use, that's all Russian all the time up there, the Eastern Fleet out of Kamchatka, that's right across the border from North Korea, Russia is right there, in the East Sea, that's a Russian Lake if there ever was one, so it's packed with Russian vessels. Russia borders with North Korea at Khasan, Vladivostok is just up the highway from there.
Why launch an ICBM at the CONUS, which is unlikely even to make it, and commit suicide in the process? If you're going to use a nuke, the Americans are right there, within spitting distance, and in Russian waters for all intents and purposes, a nuke going off there, is going to be blamed on the Americans, by the Russians, they are not going to blame North Korea, they are going to blame the USA, and then, America is going to have a much bigger problem, and there would be no nuking Pyonyang, once the Russians are at failsafe to retaliate.
