They would have to explode three nukes to cover all of the US at the size they have them now minimum... in reality, NK could not do it... An EMP attack against the CONUS by detonating in the ionosphere, is going to be treated as a nuclear attack, the retaliation would be nuclear, counterforce if perhaps not countervalue, so the NK's are no less deterred from doing that than they are deterred from launching against Los Angeles, and they are not suicidal, the people who run the regime are not looking to get a nuke down the ventilation shaft of their command bunker, so the idea that the NK's are not rational actors and so are invulnerable to deterence, is nonsense.
The notion that the United States has to go over to the offence and force a regime change, "because dem Commies is crazy, they'll nuke us without a second thought", is Red Scare hysteria to the absurd. The American deterrent is more than credible against the puny NK's, they are no more likely to nuke you out of the blue than the Russians and/or Chinese are, in fact, since the NK's are not on hair trigger alert, they are far less likely to, because the most likely scenario for that, is actually a catastrophic mistake rather than with intent.

If he was suicidal, he wouldn't spend all his time desperately trying to fend off an internal putsch, killing off his potential challengers before they kill him, up to and including his own brother, so get real.
Survival instinct, is the driving force at the center of this regime, they have no ideology, other than self preservation. As such, they are far more vulnerable to deterrence than the average, some adversaries might actually risk it all for a cause, but the DPRK is at the bottom of the list that way.
It's really not that complex, this regime has made the same calculation that all despotic regimes have made; the United States won't mess with you if you have nuclear weapons, even if you only have a handful of them, the nuclear weapon is the deterrent you need to keep the Americans at bay, if you don't have nuclear weapons, then there is a possibility of the United States attempting to force a regime change, ergo, nuclear weapons are a must, to ensure self preservation.
