We know he calls her Michael in private. That slipped at least once that I know of. I suspected for a while that she was a he because of her frame, but it could have been that she was on testosterone for a while and was slowly going through the changes in anticipation of Barrack leaving office. We also know Barrack was a homosexual in his youth (confirmed by numerous people that knew him as Barry Soetoro).
