How did the book square the circle on:

The book doesn't say that Trump was involved in any sort of collusion, it says that Don Jr met with the Russians, but that Trump actually had nothing to do with that and wasn't aware until after the fact.

The book doesn't say that Trump is running all this shit from the center, it says the opposite, he pretty much lets them all do their own thing and isn't really in the loop most of the time.

Even back in the long ago day, when elections were just as vicious personally as they are now, I don't think the attack on a president continued a year into office. Does anybody have an example?

Based on the book, there's no there there in terms of Russia collusion, Trump wasn't there, Trump didn't direct anybody to do it, and the guy's who met with the Russians, are not officials of the United States government, so there's nothing to charge Trump with, and there's not much to charge them with neither.

Who is Don jr? He's not a politician, he's free to meet with whomever he wants to meet with, what's the charge?

The book doesn't say that Trump is unfit to be President, it's people like Spicer and Conway, they were the ones saying Trump was completely out of it, but the book pretty much describes the same Trump we've known for forty years, it's just Trump being Trump, and Trump is basically pissed off because everybody around him is leaking and trying to take each other out, they're all out to advance their own agendas at Trumps expense, and that's not helping Trump, at one point he complains that Hillary has all the best people working for her, and his people are a bunch of ratfucks so to say.
