I have worked a lot of government contracts, many tied to to Federal Dollars like this project. Who has responsibility for this ?
On the Government side the Contracting Officer (followed by a bunch of other people), on the Contractor side, the Contractor who is backed up by a bonding company.
How does something like this happen, lack of Quality Control(contractors responsibility) and Quality Assurance(governments responsibility).
Contractors are required to inspect their own work while it is being performed, and document on reports to the government. The government will require the reports, such as concrete tests, typically one per truck, slump, air entrainment, 7 & 28 compressive break tests, etc. The government then reviews the QC reports and typically takes their own tests for Quality Assurance.
On the Project Management side the Contractor is to do everything by the plan and inform the government of any changes to the plans and specs. This includes construction methods and schedules...... and even personnel changes. The government will have project engineers and inspectors as well, especially with urban projects that often have inspectors that are used and sometimes unions play a roll. In this case sounds like Engineers identified cracks in the concrete 2 days before the collapse, but nothing was done. Engineers are typically not the first ones to identify a problem like this, it is usually an inspector. There were likely problems identified far earlier than 2 days prior.
I see this shit more often than I would like. People discover a problem and are afraid to confront the issue. I just describes a logical system that is designed to prevent just such catastrophic failures. If you think for a second that the Federal Government isn't deepling involved and responsible for this you are crazy. Look no further than the NTSB investigating this. The Feds don't give up 14 Million with strings attached and their people being involved all the way through. Engineers can be really fragile creatures when shit goes south and their fingerprints are on the design or construction.
I fucking guarantee you some Engineer in government authorized the improper supporting of the structure prior to completion of structural elements. Because there was some low paid Engineer Tech or Construction Manager raising a fuss that things weren't going per plan, that guy wrote it down on a daily report. I can only theorize to why the Quality Control plan was not followed, but it most like had to do with time or money, could have been concern about traffic flow, hard saying.
This is the Project Management Triangle, it is an equalateral triangle, if one side grows or shrinks, all the others are directly affected.
This reminds me of the Big Dig on a smaller scale

Hey guys, I have a new idea for a way of installing a suspension bridge. You just build the bridge on the side of the road, fit it into place without the suspension, and then add the suspension cables last.

Renderings of the project before it went up showed a tall, off-center tower with supporting cables attached to the walkway, the Associated Press reported. When the bridge collapsed, the main tower had not yet been installed, and it was unclear what builders were using as temporary supports.

Telling the feminists designing this thing that they need to build the suspension towers first is literally patriarchy. They don't need you to mansplain to them how to build a fucking bridge. Women were building bridges long before misogynist men took over society from the matriarchy.

Here's how this Affirmitive Action shit goes in Government Contracting. The Government has targets for the number or contracts or dollar value of contracts to dole out to certain demographic groups............ anyone in business has heard of a guy having his wife be teh LLC President so they can claim Small, Disadvantaged Business status. So here's where politics can really get involved. Politicians can and do influence Contracting on certain projects and what Demographic group they are set aside for. It's very likely this project was advertised as a minority and/or woman owned business. Which means straight white males may not even bid on the project. We see projects all the time that would be great projects to bid on, but a lot of firms are excluded from the get go................
